I've not heard of anyone reporting a problem with incorrect OV_HOLE returns - 
the most likely reason (by far) is that you have a hole in your ogg files (a 
'hole' in this case is some chunk of data that can't be decoded as ogg - 
maybe missing the ogg header, or with the wrong checksum, etc.). 

You can use ogginfo to check them - it'll list errors. It's part of 
vorbis-tools.
