[theora] Extension .v+ogg does work

Lets not try and be too creative.

There's no downside to being more conservative and using .vogg .oggv 
.oggvideo .ogv .theora or any other reasonable combination.

While it may be strictly permissible to use .v+ogg , it's bound to cause 
issues for some software, which may not have the most robust filename or 
url parsing. Additionally a + has to be escaped in a URL. .v%2Bogg 
doesn't look quite so cool.

There's no technical reason to try and put a plus sign into a file 
extension just for the sake of it. It will just confuse and infuriate 
people for no benefit.

I've been accepting .ogv in windows for a long time. I think .vogg is a 
good choice or even .oggvideo . .oggv is going to be unfortunate becuase 
it could be mistakenly thought to be ogg vorbis for those not in the know.
