[Stan looks sheepish.]  I didn't have CVS access at the time, but I kept the
patch.  If you look at CVS now, you'll see that the macosx support is
basically your patch.

However, between your patch and the current code, there was a rather massive
architectural overhaul of the libao guts, including support for dynamically
loadable modules.  This was a requirement for distributing binary libao
packages since some output plugins may require libraries that aren't present
on all systems.

So, while we could chuck dynamic plugins on OS X, I would rather force us to
figure out how to make them work.
