ffmpeg has little chance of producing valid Theora output depending on
the version, especially as ffmpeg's Ogg mux support is broken anyway.
Their homespun vorbis encoder is also broken. Regardless what's
happening, we have no control over the ffmpeg source base. You'd need
to contact the ffmpeg people, but we've not found them particularly
cooperative about fixing bugs.
In the meantime, you could try ffmpeg2theora, which uses ffmpeg as a
swiss-army-knife front end and then bolts that output onto our own
encoders.
