[paranoia] hidden tracks

A 'hidden' track at 'track 1 index 0' (the first track pregap, which
is where this one is) can be ripped as track 0.  Note that this won't
work for all CDROM drives; on many drives it just wont work and on
some it can, in fact, lock up the drive firmware.
