the access log should always have an entry. If it doesn't something is
wrong.
In the good case the first request is rejected (with a 401), and then
the client will do all other requests authed (mostly 2xx status codes).

That depends on the client you use. Without knowing that nobody can
answer that. It should be in the documentation of the software you use
to make the request.

This has nothing to do with the listen sockets.
