[icecast] Howto stream using icecast/ices bit rate peeled ogg files

That would be a ideal solution but strictly speaking bitrate peeling is
a todo feature.  The spec allows for it but the current encoders are not
tuned enough for doing that sort of thing well enough.  ices currently
has no support for it yet, so re-encoding is the current solution.
