[icecast] [PATCH] let ices do multiple streams of the same playlist

This patch:

1. makes ices compile in my system. As I said, the CVS repository
   looks like it was abandoned in the middle of a reorganisation.
2. Lets ices stream the same playlist at multiple bitrates.
3. Has a lot of pointless whitespace changes. I didn't feel like
   learning how to teach emacs how to blend in better. Also the code
   looks like it was written by a guy with a 150-column terminal.

Incompatible differences:
the ices:Reencode tag in the config file is now in the streams
section.

I'm not 100% sure I covered all the error conditions, I wrote this
pretty quickly.

PS: Jack, what's it take to get a patch into CVS? I have an ancient
patch for the liveice %s format-string bug (which can cause a lot of
segfaults) too. And it would be nice if the current CVS tree would
compile. I can send patches for that...
