[icecast] Icecast under cygwin?

Cygwin had too many thread issues back when I was doing the windows
port, so I did a true port.  It builds great under MSVC6.

There are a few windows specific issues that arne't worked out in the
current codebase, but icecast2 will get extensive win32 testing from the
beginning.
