[vorbis] psychoacoustic model description

There is currently no detailed description of the Vorbis psychoacoustic model 
except in Monty's head.  All the developers capable of writing such a 
description are too busy writing code trying to get 1.0 out the door.

Once 1.0 is released, you should start to see such "human-level" documents 
appearing.
