[vorbis] new ogg stream

Erik,

Cascaded codebooks were an rc1 feature.  That was one of the big things
for peeling.

Even if we only got 20%, it's an awesome feature, since that allows us
to fine tune streams on the fly.

But I leave it up to Monty to provide an official answer.
