[vorbis-dev] c++ comments in vorbis source

Monty,

Updating the MacOS build files, I found two //-style comments in the 
vorbis source. I believe we have a general policy against this, and 
since they look like dead testing code, should be removed for release 
anyway. Can you do so? I don't want to step on your code.

        envelope.c line 248 and
        psy.c line 1020

Note that I also went ahead and removed a number of unimplemented 
prototypes from ogg.h; I'm not sure what the intention was there. See 
the commit message for details.
