The idea is that initial connect will get codebooks seperately, but on
stream-change, the headers will be sent in-line.
Vorbis requires the headers, and can't tolerate any corruption, so it is
neccesary to ensure that clients are able to reliably obtain the headers.
That's why this is an 'open issue' at this point.

That's been suggested, considered, and I believe rejected. It increases the
bitrate continuously (rather than just on connect, for instance), and given
an unreliable stream it could take a LONG time to connect.
