[vorbis-dev] Merge of current Monty branch

I just merged monty_branch_20000524 onto the vorbis mainline.  This has
numerous quality improvements and fixes... upshot should be the best sounding
Vorbis yet with a nominal average bitrate of 110-128 kbps.

I have a few API changes and mode descriptions left to write for beta release 
which we're currently shooting for on the 20th.  The default encoding mode is 
based on the mode description I just merged-- try it out and let me know if 
and where it falls flat.
