Tonight's checkin:

I fixed a silly but serious mdct bug; my last checkin was broken.  Make sure to
update from CVS to get a working copy.

decoder_example now handles chained files, but this isn't the source example
that Tony was looking for.  I have that mostly done and expect to check it in
in the next day or two.
