Yes, the clause after the "Otherwise" was meant to clarify d.ii, not d.i.B.

Both of these have been fixed in svn r14614. I also added a column to
table 7.19 for mode scheme 7, for clarity, and fixed the number of bits
required for mi (it was 32, it should be 3).
