[Icecast] icy-metaint in header and in stream data

I've never seen such a stream. I've never seen a client or server
implementation that could handle that being done. To the extent that
there's any sort of 'standard' this would clearly violate it.

I'm pretty sure nobody ever sends it in the response data.
