[foms] <video> ActiveX plugin

  Hi,

Here is a short summary of the issues raised at FOMS regarding the
<video> ActiveX plugin:

* xmlns change from "http://www.w3.org/1999/xhtml/video" to
   something else, something like "http://www.xiph.org/video"
* packaging as a cab download, this would provide the same user
   experience as the cortado java plugin
* installation and use of the plugin also for non elevated user accounts
* addressing the performance issue by creating a custom video renderer 
filter

Since I'm doing this from memory I might have missed something, please
do comment :-)
